REGISTRATION FOR OUTDOOR SOCCER

 

Registration will take place at the Pembroke Soccer Club, located at the Knight’s of Columbus Hall, 170 Ellis Avenue in Pembroke and will be held on the following dates:

Tuesday, February 26th, 2008        6:30 p.m. to 8:00 p.m.

Thursday, February 28th, 2008      6:30 p.m. to 8:00 p.m.

Saturday, March 1st, 2008                1:00 p.m. to 4:00 p.m.

Monday, March 3rd, 2008               6:30 p.m. to 8:00 p.m.

Wednesday, March 5th, 2008         6:30 p.m. to 8:00 p.m.

 

Please ensure you attend on one of these dates to register. Registrations received after March 5th, 2008 will be considered late and the late fee will apply (see fees below). Children registered after the dates listed above will be put on a waiting list and placed on teams if space is available. The number of teams per division will be based on the number of registrants received on the above dates. Please make sure you attend so your soccer player(s) will not be disappointed.

 

ELIGIBLE AGE GROUPS FOR 2008:

 

Under 6   born between January 1, 2002 and December 31st, 2003

Under 8   born between January 1, 2000 and December 31st, 2001

Under 10 –born between January 1, 1998 and December 31st, 1999

Under 12 –born between January 1, 1996 and December 31st, 1997

Under 14 –born between January 1, 1994 and December 31st, 1995

Under 16 –born between January 1, 1992 and December 31st, 1993

Under 18 –born between January 1, 1990 and December 31st, 1991

 

We apologize for the number of pages in the registration form; however this is an OSA requirement. PLEASE NOTE: ALL THREE PAGES OF THE FORM MUST BE COMPLETED AND SIGNED IN THE APPROPRIATE LOCATIONS TO BE VALID TO BE REGISTERED WITH THE OSA DATABASE . Additional forms can be downloaded from our website at www.pembrokesoccer.com. The Player Registration Number does not have to be filled out on the form. If you have been registered with the Pembroke Soccer Club in the past, you can send registration by mail along with your payment, postmarked by 5 March 2008.  Any new players will have to register at the office on one of the dates listed above. New players will require proof of their date of birth (birth certificate, health card, any other official document).

 

SOCCER REGISTRATION FEES:

 

The cost of registration for the first two children from each family is $95 each and $65 for each additional child from the same family. All late registrations will cost $125 each for the first two children in the same family and $90 each for additional children. This will be strictly adhered to. Competitive fees will be $60. This fee does not have to be paid until the player has been selected for a team, but must be paid before the first tournament.

 

 

CITY OF PEMBROKE USER FEES:

 

Please note that players living outside the City of Pembroke or the township of Laurentian Valley must pay a user fee of $18 per child. It is the responsibility of the player’s family to submit user fees directly to the City of Pembroke. Fees can be paid in person at the Recreation Department, located at the PMC, or a cheque can be mailed directly to:  Attention: Joanne Nagora

EQUIPMENT:

 

Players are required to wear shorts, soccer shin pads, socks and cleats (or sneakers). Jerseys are provided by the Club and must be returned at the end of the season (except for U6 & U8 Tim Hortons).

  

ATTENTION : PARENTS / STUDENTS

Volunteers Needed

If you would like to volunteer as a Coach, or an Assistant, please indicate your interest on the registration form in the “Parent/Guardian Emergency Info” area. (Coaching Clinic will be offered)  Students looking for community volunteer hours will be welcome. Call the Pembroke Soccer Club @ 613 732 0925 or contact Cheryl Hanniman @ 613 732 1229.

There are many “small” tasks we need volunteers for, so if you can help, please do! We’d appreciate everyone’s support!  

 

COMPETITIVE TEAMS:

 

The Pembroke Soccer Club will again have competitive soccer teams. Players 10 years of age and older are invited to try out. This is an opportunity to play at a higher caliber of soccer. Players 15 years and older are not required to play in the House League, but are still required to pay the House League fee in

addition to competitive fee if they make the team. Those who wish to play competitive soccer only, must download and submit a completed and signed “competitive” registration form which is available on our website (www.pembrokesoccer.com). Please indicate on it that you only wish to play competitive. Those who wish to play both are required to play for the full season. New players are required to provide proof of their date of birth and a recent photo at the time of registration.

 

Players 14 years of age and younger who wish to try out for a competitive team must submit two completed and signed forms: (1) a House League Registration Form and (2) a Competitive Registration Form.

Note:  Teams are dependant on availability of certified competitive coaches and number of players.
